About:

The Fall Prevention Program provides supportive phone calls and resource connections to help make your home safe and prevent future falls. We want to provide education and support to improve your well-being.

Home-Visit:

An initial home-visit is available in Sioux Falls, South Dakota only. Phone call-based services are available State-Wide. This program is provided at no cost.
