Program Information
Provides skilled nursing facility care tailored to short-term rehabilitation and long-term care services. Also provides respite care services and a secured memory care unit with staff trained to care for those living with Alzheimer's and Dementia.
Eligibility
All persons who meet criteria for skilled nursing care and agreed upon by their primary care provider.
