Transition to Success
Transition to Success™ (TTS) is an evidence-based program that treats poverty as a condition — not a personal failure. Developed by Dr. Marcella Wilson, TTS addresses the root causes of poverty by targeting the social determinants of health: unstable housing, food insecurity, limited education, lack of healthcare and more. Rather than offering temporary fixes, TTS builds a foundation for lasting change.
How does Transition to Success Work?
Every TTS participant is paired with a trained coach who helps them assess their situation and build a personalized care plan called the Map of My Dreams. Together, you’ll set goals across areas like housing, employment, education and health — and take real steps toward achieving them.
Coaches are trained in trauma-informed care, motivational interviewing and cultural competence. They’re not just guides — they’re trusted partners who help you access every available resource in the community.
The program also includes financial literacy, mentorship and volunteering opportunities to build confidence and long-term stability.
